Under the rule of the military government, the western part of Myanmar has been covet by the army for many years due to its rich resources. Children there are often forcibly recruited by the army, and massacres of civilians have become the entertainment of some soldiers... In order to enter the area, a team of church volunteers provides humanitarian assistance to refugees, and finds the reclusive Rambo (Sylvester Stallone) in Thailand, hoping that the latter, who is familiar with the river, will provide help. Rambo believed that there was no possibility of change in the current situation and refused to lead the way, but was finally moved by the sincerity of volunteer Sarah (Julie Benz) and sent them to serve him. Unexpectedly, this volunteer team was attacked by the army in the village, and all the survivors were kidnapped. The church learned that the volunteers were in danger, organized mercenaries to rescue them, and once again entrusted Rambo to lead the way. This time, Rambo's attitude changed. He led the mercenaries to sneak into the Burmese military camp and once again transformed into a hot-blooded soldier.
